Instructions
for submitting an Application
A complete
application for the 2006-2007 AICHE Minority Scholarship Award will
contain a copy a copy of
each of the following items (each letter should contain an original
signature):
1.
From the student:
a)
A completed application
form (this form)
b)
A letter not to exceed 300
words outlining the following:
*
College or university
chosen to attend; when and why.
*
Reason (s) for choosing
science/engineering.
*
Possible career choices
that may be of interest.
c)
List of school, job and/or
civic activities and positions held.
d)
Official transcript of high
school grades
2.
From the parent or
guardian:
a)
A letter verifying
financial need. Please include a list of financial resources for
educational support.
3.
A letter of recommendation
from the high school counselor, math teacher or science teacher
containing, but not limited to, the following:
a)
Verification of the
student’s GPA and graduation date.
b)
Verification of the high
school senior class average grade.
(If
confidential, High School Counselor statement is required).
c)
Confirmation of minority
group of student.
d)
Information about the
student’s school, job and/or other civic activities.
